Primary health care survey

Horizon is committed to improving access to primary health care by shifting from a single primary care provider (family doctor or nurse practitioner) to a collaborative approach. 

Remember: primary health care is the first place you go for health services. 

This collaborative approach will see you cared for by a team of health professionals, that we are calling Family Health Teams. As we transition to this model, we want to understand your experiences with our primary health care services. Your feedback will help us identify what’s working well and where improvements are needed to guide us as we establish these teams across Horizon.
