Language Experience Survey

Good communication is key to delivering quality health care.

We are inviting patients and families to complete a brief, anonymous survey, about their experiences accessing health care in their preferred official language (English or French).

Your responses help us understand how we are doing and identify areas for improvement.

After completing the survey, you can also choose to share more about your experience. Sharing your story is optional, but patient stories help us better understand the impact of language in health care and build awareness among our teams.

We are especially interested in hearing about moments such as:

• Trying to explain symptoms or pain
• Understanding diagnoses or test results
• Receiving instructions about medication or follow-up care
• Being asked to use a family member or friend to interpret
• Feeling confident — or not confident — that you were fully understood

What sharing your story involves

• A short conversation (by phone, video, or in person — your choice)
• You choose what you want to share
• You can remain anonymous
• You can review any quotes before they are used
• You can withdraw your participation at any time
• Your decision to share a story will not affect your access to health care or services in any way.
