Palliative Care Patient and Family Experience Survey

Thank you for taking time to complete this short survey. Your feedback is important to us. It helps us to know what we are doing well to support patients and their families, and what we need to change. 

We are asking all patients and their families who are currently receiving care from our Palliative Care Team to complete the survey. It focuses on the care and services we provided to you this past week. This survey is anonymous.  

If you have specific concerns you wish to share immediately please ask to speak with the Nurse Manager on your unit or contact Patient Representative Services Toll Free 1-844-225-0202. 

Please note: to make it easier to read, we will simply use you when asking questions (rather than asking you or your loved one), but we welcome feedback from both patients and their families.
