May 11, 2023 update: the findings from the survey were compiled into a report with recommendations and were submitted to Horizon’s Executive Leadership team in March 2023.
Dec. 21, 2022 update: The data has been collected, reviewed and integrated into a draft report along with information from staff surveys, documentation reviews and an external environment review.
The next steps include finalizing the report and making recommendations. The plan is to submit the report to the Horizon Health Network early in the New Year.

Thank you for taking some of your valuable time to consider our survey. It is critical that our health care system constantly review and improve all its processes. The COVID-19 pandemic has challenged the system in many ways and the likelihood is more pandemic-like incidents will happen in the future.
This survey is part of a larger project that is looking at Horizon Health Network’s (Horizon’s) response to the pandemic from many different viewpoints. The following is a list of the components that have been completed, to date:
a literature review of what has happened across the country and around the world;
a document review of Horizon’s committees and workgroups;
interviews with senior leaders at Horizon;
focus groups with staff.
This survey is the next step. It is built on principles from the United Nations Office of Internal Oversight Services’ criteria for evaluating COVID-19 responses and adapted for a health care environment. The goal is to use all this information to recommend ways to make improvements going forward based on what has been learned from our collective experience.
