The Diabetes Education Centre (DEC) patient experience survey was developed to solicit direct patient and support person feedback. The opportunity to provide feedback through this survey will allow patients and their support person(s) to be engaged in directing the care they receive. This feedback will be used when reviewing services and care provided and to make positive service delivery changes to support quality patient care.

We want patients to know they have a voice in the service and care provided to them. Feedback from patients is the only way to assess existing services and patient needs and make meaningful changes to improve care and services.

    This survey will take about three to five minutes to complete, and you will be asked how you feel about the quality of care you received at the Diabetes Education Centre at Horizon.

    The results of this survey will help our organization improve the quality of care that you and other patients receive.

    Privacy and confidentiality: All information collected will be anonymous and voluntary, and responses will be stored in a secure database.
